Seven Steps to Well Permit Approval

  1. Complete the "Application for Well Construction/Deconstruction", including A SKETCH as required on the back of the application form. Be sure to include the relative location of your chosen well site, perc tests, house location, etc. Missing information will delay issuance of the permit.
  2. The application must be signed by the property owner only.
  3. Enclose a check in the amount of $50 written to the Town of Williamsburg.
  4. Mark the entrance of the lot with a LARGE number, lot number, or your name so I can find it.
  5. Mark your chosen well site with a LARGE painted stake or flag marked "well". Your well driller and you should determine this location together.
  6. Mark the location of each percolation test on the property with orange flag tape.
  7. Mail the application to: Health Agent, PO Box 447, Haydenville, MA 01039.  

Then What?

I will visit the property. The site visit will be incorporated into my travel schedule with other field work in your town as soon as I can get there. If additional visits are required due to a lack of information on this application, or due to incorrect measurements of the required setbacks listed on the attachment, you will be charged an additional $50 for each visit. Therefore please measure all setbacks to be sure they are met.

I will mail or fax the permit to the name and address on the application. I will also fax a copy of the permit to the well driller listed on the application. Approval may take up to 30 days. Please be patient.

The well may be drilled after you receive the permit. Afterwards, the well driller will take a water sample to a qualified water lab for testing. The lab report will be returned to you. IMPORTANT: you must forward a copy of the laboratory report to me at the above address immediately. Otherwise, your building permit cannot be issued.
